AUTHORIZATION FOR BACKGROUND CHECK
Below is an Authorization for Release of Information, which we will need you to complete, sign, and return to us. The search process can take some time, so we would appreciate it if we could receive a copy of this form with your application.
Please note that the search request form requires a notary signature as well. Please assure that this is completed prior to submitting it to us. We plan to complete some of the following searches: Nationwide Criminal Search, Credit Report, Bankruptcy Search, and Education Verification, for those who progress to the final phases of the process.
For a national background search to be conducted by the FBI, we will need additional information as this search is based on an individual's fingerprint submission. The fingerprinting must be completed by you and submitted along with your Authorization for Release of Information. A fingerprint card is attached for your use. Please complete the information required on the fingerprint card, and take it to your local law enforcement agency to have your fingerprints taken. Without the fingerprints and release form, your application will be incomplete. If you will be emailing your application, we will accept a scanned or faxed copy of these documents to start the process. We require, however, that the original documents are submitted to the Succession Planning Committee under separate cover via US Mail no later than December 20, 2009.
Please know that we understand the sensitivity of the information that we are requesting. We assure you that your information will remain confidential, and that every precaution will be taken to preserve your privacy.
